Requirements for Adult - Lost Passport

A Lost passport applications will be treated as NEW APPLICATION,

Additional requirements:

• Affidavit of Loss in English

Additional requirements may be required depending on circumstances of the applicant. Please refer to the section below.

1. In case of a Lost VALID/EXPIRED Passport,

• Original Police Report in English

2. In case passport has been mutilated or damaged - will be treated as NEW APPLICATION,

• Affidavit of Explanation
• Applicant must surrender mutilated/ damaged passport

3. In case applicant is a Dual Citizen,

• Government Issued IDs (Refer to Valid Philippine Government Issued IDs or their secondary citizenship/ residence counterparts)
• Original Identification Certificate, Order and Oath of Allegiance issued by the Philippine Embassy Canberra or the Bureau of Immigration

4. In case applicant is a Naturalized Citizen,

• Original Identification Certificate of Naturalization from Bureau of Immigration

5. In case applicant is a Filipino citizen by Election,

• Affidavit of Election of Philippine Citizenship
• Original Identification Certificate of Election from BI
