Report of Birth (ROB)

If you are from the Australian Capital Territory, Queensland, Western Australia, Northern Territory, Vanuatu, Nauru, and Tuvalu, please see below information:

The Report of Birth is a declaration of the person furnishing information on the birth abroad of a Filipino child. It is important for Filipinos permanently residing abroad to promptly register with the Embassy the birth of a child in order for such a birth to be registered with the Philippine Statistics Authority (PSA).

When a child is born abroad to parents who are both Filipino citizens or to one (1) parent who is a Filipino citizen, i.e. one who has not been naturalized as a citizen of a foreign country, the child’s birth must be immediately reported to the Philippine Embassy or Consulate General which exercises jurisdiction over the place of birth. The Report of Birth (or PSA-issued Birth Certificate, if ROB was issued more than six months ago) is also one of the requirements should the child apply for a Philippine passport.

After 6 months - 1 year from the registration of birth at the Embassy/Consulate, the PSA can issue the official birth certificate upon the request/application of the parents.

Please take note that the consular officer may require additional documents upon assessment of the application, depending on the circumstances of the applicant’s birth, marriage or death. Factors such as citizenship, previous civil status and others may affect the status of your application. REQUIREMENTS:

  1. If sending by mail, include a covering letter (handwritten or typed), addressed to the Civil Registry Section, indicating service/transaction requested and telephone number and email address for contact details.
  2. Four (4) original duly accomplished and originally signed Report of Birth Forms (either type written or legibly handwritten) by any of the following:
      • Filipino parent or Filipino parents;
      • the person whose birth is being reported if over 18 years of age;
      • attending physician or attending nurse or midwife; or
      • the foreign parent (only if none of the above can accomplish the report).

      • All four (4) Report of Birth Forms must be NOTARIZED by either the Philippine Consul, Notary Public or Justice of the Peace.

        Note: The form must be accomplished neatly. No erasures please as this will be the permanent record. The Embassy has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.

  3. Original Birth Certificate of the child with registry number issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/territory where the birth was registered plus four (4) photocopies. (Note: Original will be returned together with the official copy of the Report of Birth of your child.)

  4. Original PSA issued Marriage Certificate (or original Australian Marriage Certificate with registry number) of parents plus four (4) photocopies . (Note: Original will be returned together with the official copy of the Report of Birth of your child.)

  5. Four (4) photocopies of the data page of valid passports of parents duly certified by the JP, or other forms of identification, (i.e. driver’s license) duly certified by the JP, if valid passport is not available.

  6. ADDITIONAL REQUIREMENTS

    For late registration
    • If reporting 1 year after the child’s birth, an Affidavit of Late Registration of Birth is required. The Affidavit should be properly filled up and NOTARIZED by either the Philippine Consul, Notary Public or Justice of the Peace.

    • If reporting 5 years after the date of birth, a recently-issued original PSA certificate of no report of birth record is required.
    If legitimated (if parents were married after the child’s birth) If illegitimate and Using Mother’s Maiden Surname
    • One (1) original PSA authenticated birth certificate of mother plus 3 photocopies. (Note: Original will be returned together with the official copy of the Report of Birth of your child.)
    If Illegitimate and Using Father’s Surname
    • Four (4) original Affidavit of Admission of Paternity (AAP) duly executed by the child’s father, to be signed before a consular officer at the Embassy, if filed in person.

    • Copies of father’s valid passport and the unregistered birth certificate of the child are required.
    • If the father is not in Australia, this document must be notarized and registered either in the Philippine embassy or consulate in the country where he resides or if he is in the Philippines by a notary public and it will be registered by the local civil registrar where the birth of the child is to be registered.

    • Four (4) original Affidavit to Use Surname of the Father (AUSF), executed by the child’s mother, to be signed before a consular officer at the Embassy, if filed in person.

    • Copies of mother’s valid passport and the unregistered birth certificate of the child are required.
    • If the mother is not in Australia, this document must be notarized and registered either in the Philippine embassy or consulate in the country where she resides or if she is in the Philippines by a notary public and it will be registered by the local civil registrar where the birth of the child is to be registered.

    • Certificate of Registration
    • The Embassy will issue the Certificate of Registration upon submission of the Affidavit of Admission of Paternity (AAP) and the Affidavit to Use Surname of the Father (AUSF)

    If the Filipino parent is already a citizen of Australia after the birth of the child:
    • Four (4) certified true copies of the Australian Certificate of Citizenship
    • Four (4) certified true copies of the old Philippine passport
    If the Filipino parent reacquired/retained his/her Philippine citizenship after the birth of the child:
    • Four (4) certified true copies of the documents pertaining to the re-acquisition/retention of his/her Philippine citizenship.
    • Please note that in addition to the above-mentioned requirements, other documents might be required when deemed necessary by the Consular Officer of the Embassy.

    Filing can be done through either of the following:
    1. IN PERSON (personal appearance is required if the child will also apply for a Philippine passport).
    2. Please call the Registry Section at (02) 6273 2535 extension 233 to set-up appointment)

    3. BY MAIL
    4. Send all the requirements and applicable additional documents along with the following:

      • AUD 45 payment for the Report of Birth payable by a postal money order or bank cheque to the Philippine Embassy. If paying by credit card please provide the credit card name, number, expiry date and cvv number at the back of the card).
      • AUD 45 payment for Affidavit of Delayed Registration of Birth, if applicable.
      • AUD 45 payment for Affidavit of Admission of Paternity (AAP), if applicable.
      • AUD 45 payment for Affidavit to Use Surname of the Father (AUSF), if applicable.
      • AUD 45 payment for the Certificate of Registration to be issued by the Embassy for AAP and AUSF, if applicable.
      • Pre-paid express return envelope (large size) with return address.
      • Please provide your cell phone number and email address to allow the Embassy to communicate with you regarding your application.
