Application Requirements

Applicants may apply for the ePassport at any of the following: the Department of Foreign Affairs (DFA) in Manila, the DFA's regional offices throughout the Philippines, Philippine Embassies and Consulates abroad and mobile passport services in the applicant's areas. (Applicants in Australia may apply at the Philippine Embassy in Canberra or the Philippine Consulate General in Sydney.)

While the application for a Philippine ePassport is lodged with the Embassy, the passport is produced in Manila. It takes an estimated 6-8 weeks for the new passport to be ready for release.

The Philippine Embassy therefore advises all Philippine passport holders, whose passports are expiring within the next six months, to renew their passports if they intend to travel within the aforementioned period.

Furthermore, Philippine passport holders are advised to secure their new passports prior to booking their flights.

Passport application hours are 9:00 a.m. to 12:00 p.m. and 1:00 p.m. to 4:00 p.m. on Mondays to Fridays. The Passport Section is closed on public holidays. No appointment is necessary. Applicants are attended to on a first-come, first-served basis.

Reminder: Applicants are reminded to bring photocopies of requirements. The Embassy has no photocopying services.

A. FOR FIRST TIME PASSPORT APPLICANTS

General requirements:

  • Personal appearance
  • Duly accomplished application form. ( NEW REGULAR PASSPORT APPLICATION (Adult) )
  • If born in the Philippines, original and photocopy of Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or original and photocopy of the Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is reuired when entries in PSA Birth Certificate are blurred or unreadable
  • If born abroad, Report of Birth issued by the Philippine Embassy or Consulate where birth was registered
  • No need to submit a passport size photo
  • Valid picture IDs and supporting documents to prove identity (please refer to List of Acceptable IDs and List of Supporting Documents)
  • Self-addressed prepaid registered post return envelope

Additional Requirements:

1. For Married Women,

  • If married in the Philippines, original and photocopy of Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or original and photocopy of the Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Marriage Contract from the LCR is required when entries in PSA Marriage Contract are blurred or unreadable
  • If married abroad, Report of Marriage issued by the Philippine Embassy or Consulate where marriage was registered

2. For Minor Applicants (below 18 years old),

  • Duly accomplished application form. ( REGULAR PASSPORT APPLICATION FORM (Minor) )
  • Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
  • Photocopy of Marriage Certificate of minor’s parents
  • Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents (Please refer to List of Acceptable IDs)

3. If minor is legitimated by subsequent marriage of parents,

  • Authenticated Birth Certificate from PSA must include annotation regarding new status as legitimated and the full name of the child

4. If minor is illegitimate but acknowledged by father,

  • Birth certificate from PSA reflecting surname of father with Affidavit of Acknowledgement and Consent to use the surname of father

5. For Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225,

  • Identification Certificate
  • Oath of Allegiance
  • Order of Approval
B. FOR RENEWAL OF PASSPORT APPLICANTS

General requirements:

  • Personal appearance
  • Duly accomplished application form. (RENEWAL REGULAR PASSPORT APPLICATION FORM (Adult))
  • No need to submit a passport size photo
  • Most recent expiring or expired passport (original and photocopy of bio data page)
  • Photocopy of Australian visa page or Visa Grant Notice or VEVO (Visa Entitlement Verification Online)
  • Self-addressed prepaid registered post return envelope

Additional Requirements:

1. Brown Passport or passports issued prior to 01 May 1995, Green Passport or passports issued after 01 May 1995 and Machine Readable Passport (MRP) issued from 2007 to 2010

  • Original passport and photocopy of data page and last page
  • If born in the Philippines, original and photocopy of Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable
  • If born abroad, Report of Birth issued by the Philippine Embassy or Consulate where birth was registered
  • For women married in the Philippines, original and photocopy of Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or original and photocopy of the Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.   • For women married abroad, Report of Marriage issued by the Philippine Embassy or Consulate where marriage was registered
  • Valid picture IDs and supporting documents to prove identity (please refer to List of Acceptable IDs and List of Supporting Documents)

2. Electronic Passport (E-passport)

  • Original e-passport and photocopy of data page and last page of passport

3. For married women who would like to use their married surname,

  • If married in the Philippines, Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Marriage Contract from the LCR is required when entries in PSA Marriage Contract are blurred or unreadable
  • If married abroad, Report of Marriage issued by the Philippine Embassy or Consulate where marriage was registered

4. For married women who would like to revert to their maiden name,

  • If born in the Philippines, original and photocopy of Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable
  • If born abroad, Report of Birth issued by the Philippine Embassy or Consulate where birth was registered
  • If already widowed: Death Certificate of the deceased husband
  • If marriage is annulled: Certified True Copy (CTC) of annotated PSA Marriage Contract and Court Order effecting the annulment
  • If divorced:
    • Certified True Copy (CTC) of the first page and the dispositive portion of the judgment on divorce secured by foreign spouse authenticated by the Philippine Embassy or Consulate where the divorce is obtained
    • Original and CTC of Philippine Court recognition of foreign divorce decree
    • PSA Marriage Contract with the annotation of the Divorce Decree

5. For minor applicants, requirements for first time minor applicants are still prescribed, in addition to the passport of minor

6. For Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225:

  • Identification Certificate
  • Oath of Allegiance
  • Order of Approval
C. FOR REPLACEMENT OF LOST PASSPORTS

General Requirements:

  • Personal appearance
  • No need to submit a passport size photo
  • Submit same documents required for first time passport applicants
  • Photocopy of Australian visa page or Visa Grant Notice or VEVO (Visa Entitlement Verification Online)
  • Self-addressed prepaid registered post return envelope

Additional Requirements:

1. In case passport was lost and cannot be found,

  • Submit Notarized Affidavit of Loss/Statutory Declaration (with detailed explanation on when, where and how passport got lost)
  • If the lost passport is still valid, submit Police Report
  • Photocopy of first page of lost passport (if available)

Passports declared lost or stolen are automatically cancelled and can no longer be used even if found

2. In case of mutilated or damaged passports,

  • Submit Notarized Affidavit of Mutilation/Statutory Declaration (with detailed explanation on when, where and how passport got mutilated or damaged)
  • Original and photocopy of first and last page of mutilated or damaged passport
LIST OF ACCEPTABLE IDS (At least 1 of the following),

Government-issued picture IDs such as the following:

  • Digitized SSS ID
  • Driver’s License
  • GSIS E-card
  • PRC ID
  • IBP ID
  • OWWA ID
  • Digitized BIR ID
  • Senior Citizen’s ID
  • Other acceptable picture IDs such as the following:
    • Old College ID
    • Alumni ID
    • Old Employment IDs
LIST OF SUPPORTING DOCUMENTS (At least 3 of the following),
  • Voter’s ID
  • NSO Marriage Contract
  • Land Title
  • Seaman’s Book
  • Elementary or High School Form 137 or Transcript of Records with readable dry seal
  • Government Service Record
  • NBI Clearance
  • Police Clearance
  • Barangay Clearance
  • Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant such as the following:
    • NSO Birth Certificate of child / children
    • SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
    • Income Tax Return
    • Voter’s Certification, List of Voters and Voter’s Registration Record (for newly issued VRR, please attach receipt)
    • School Yearbook
    • Community Tax Certificate
